Dear Stone Valley Middle School Parent,

Thank you for volunteering to chaperone and drive your child and their classmates.  Without your time and supervision, we could not have these school activities. 

For the safety and well being of all students, the district requires the following paperwork to be kept on file for all Chaperones and Drivers:

VOLUNTEER CLEARANCE FORM

The district requires each volunteer to complete a SRVUSD Volunteer Orientation.   You can either make an appointment with Denise True – Office Manager at Stone Valley Middle School OR view a video at home by going to the District website:   http://www.srvusd.net/parents/volunteer_video   this video takes about 10 minutes to view, and at the end requires you to print off a Volunteer Clearance Form.  This form will need filled out and turned in with the other forms listed in this letter.

This form can be kept on file for the three years that your child is at Stone Valley Middle School.

Personal Automobile Use Form

Please make sure to attach the required Driver’s License and current Auto Insurance information.  These copies will need to be resubmitted upon expiration.

1.      Go to San Ramon Unified School District webpage at: http://www.srvusd.net/

2.      Go to Parents tab

3.      Click on Volunteer Clearance tab and click HERE to watch video

4.      After watching video click on Volunteer Clearance Form

5.      Print form and bring to Stone Valley office with a copy of your driver’s license.
